Surface Water Monthly Operating Report (SWMOR Version 14, Excel 2007)

Reporting form for public water systems that treat surface water or groundwater that is under the direct influence of surface water (GUI). It contains information used to determine a water treatment plant's compliance with treatment technique, monitoring, and reporting requirements.
